Vital Records

All births and deaths that occur in Ripley County are recorded at the Health Center. 

~  Missouri Births after 1920 and Deaths after 1980 ~

Certified copies of birth certificates for a person born in the state of Missouri after 1920 and death certificates in the state after 1980 can be obtained at Ripley County Health Center. 

COST OF VITAL RECORD CERTIFICATES

• Birth Certificates are $15.00 each (per copy).
• Death Certificates are $14.00 for the first copy and $11.00 thereafter.

IN PERSON

Our staff can help you with all the necessary paperwork if you come by the Ripley County Health Department. You will need to bring:

1. photo identification
2. sufficient payment (personal checks are accepted)

BY MAIL

You can also mail us a request for vital records but the request MUST BE notarized. Just print out the form, fill it out, get it notarized by signing before a notary public, then mail it in with your payment along with a copy of your drivers license.
